Does anyone have a "work to hire " sample agreement?

I'm looking for a sample retainer agreement (work to hire) between a production company and myself for exclusive movies that I wish them to shoot for me.

I could only find on google a work to hire agreement between a production company and a model, I can't find an agreement between a production company and a webmaster wishing to obtain exclusive movies.
