Is there a way to automatically make all links go through out.php?

no. you have to edit the theme and edit a system file

it would take some editing of the wordpress code, but yeah you can do it

This is a feature that I've been waiting to come along not just for WordPress, but general CMS as well. I wonder when we'll see it by default on these scripts and software.
