Request Tracker is pretty much the de facto standard in open source circles in Silicon Valley.

Check out Request Tracker. Can be a pain to install, but works quite well.

There used to be a free one "perl desk" or some shit. You had to pay $75 to remove thier link at the bottom of the pages if you wanted, otherwise I think it was free to be modified.
