Looking for school report style webpage template
 

I'm planning on adding some reviews to my niche site and would like to use a 'school report' style page which allows headings, comments and a 'grade' for each heading. I haven't been able to find any decent ones online. Anyone know any available (free or modest cost) or could give me a price for putting one together?
