Just added this page:

As you can see the join form is right on the page.. it is completely dynamic. You can choose any model on our site and it will generate the page with her info. It only really looks good if the model has a lot of pics submitted but this makes it very easy for people since they pretty much do these kind of pages and typically link to our join forms anyway. We figured we'd just put the join form on the page for one less step for the surfer
