Alternative to htaccess?
Anything database driven that also protect everthing, including images in a directory?

Nothing I would trust. mod_rewrite is pretty much the defacto standard for that.

php can do it, membershipclientpro.com has a script for it.

write your own php, but mod_rewrite with mod_geoip if you want to do some extra cool things.
