I need a email script that collects surfers email's.

I need one of those email scripts that is in the form of a box located at the bottom of paysites, where the surfer puts in there email and it collects it. Where can I get one?

I don't, so I have to have a database? Sorry, I am not good with scripting and coding, is there a script with some simple instructions?
