anyone have a link to a free one that will works and can be easily customized with various fields, like: name, email address, reason(s) for sending email, and a text box for inserting their comments etc.

Sorry, I'm in a lethargic mood to try and dig out some free scripts, but if you're needing something personalised, you'll need to know php to alter the code to your tastes. If that's so, then you know php to write such a simple code.
