looking for a good form script
 

im looking for a good form script, php or cgi. it just needs a few fields for ppl to enter their email, name and phone number. also needs a function where if someone leaves a certain field blank, it won't accept it unless the person fills it in. and a flooding function, if ppl try to fill it out 1000 times over. i've heard you can use htaccess for that tho? any suggestions? free or cheap prefered.

what do you want the script to do ? email you the info or log it ??

i could hook you up with a simple image verification to add onto that script if you want

that's pretty easy to code if u know some php
