Official 2257 forms

where can i obtain these forms? i know some people just make a few copies of their models ID's and put a 2257 statement on their content, but i heard there are official forms, where can i get them?

what the hell is a 2257 form? If you have a picture of the girl's ID, that's a "2257 form"...

What you want is a release form. That covers you civilly. the 2257 covers you Criminally.
