My ex's birthday is coming up and I was thinking about giving her a call just to say happy birthday... we havn't really talked since we broke up.

Everyone likes to be thought of on their birthdays, I say call her :)

Girls will turn a thoughtful thing like calling on her birthday into something cheap and discusting like, you only called her to fuck her.

Depends on what kind of break up you both had. If you really haven't talked in a long time. It may be better to leave well enough alone. But depends what kind of "feelings" you are having.
