What are the top 5 questions you ask new models when your Interviewing them?

I'm not sure about the other 4 questions... but #1 is "how old are you?"

i don't do any of the screening myself, i only filter out the ones whose pics are passed along to me for inspection

then i ask the agent if she does x, y, z etc...

1:How old are you?
2: Do you have all your current tests?
3: How much are you?
4: What do you and don't you do?
5: How much will it cost to get you to do what you said you wont do in question 4?

1. What's your name?
2. Do you have a job?
3. What will you do?
4. What experience do you have?
5. Do you have your IDs with you?

1: Are you at least 18 with government IDs
2: What are your limits as far as working with girls, men
3: When are you available to shoot
4: Have you done any adult work before (if so do you have a current test)
5: Do you know any friends that want work
