Something I've always wondered about merchant accounts....is there any kind of minimum monthly volume required to maintain the account?

Typically the bank will have a $10-25 monthly minimum in processing fees so if you do not acquire that much in charges, they will charge you anyway. Does that make sense?

Is it more difficult to get a merchant account with an offshore bank, than a US bank?

(this is for a mainstream online business, not adult)

It depends where you are located. If you are in the US, you should surely get a US account. We set mainstrwam US accounts in 24 hours typically. US accounts are cheaper too.
