Aircraft/airline fanatics, some cockpit tapes and vids inside
Got some cockpit tapes and vids if you are interested.
Also got the full transcripts off these flights. Most of these accidents are caused by crew failure, you can read the info on every link. AF90 Cockpit tape. Captain used thrust reverse to assist the push back because it was snowed in. The engines sucked up alot of snow and the EPR (Engine Power Reading) was frozen causing the pilots to read a take-off power engine setting (92%) while the engines wheren't even at 50%. You can hear the F/O warning the captain a few times that it looks "real cold" and "no, that's not right" but the captain keeps overiding the F/O with "yes it's right, look these is 80 (indicating that they are at 80 knots)" The copilot was doing the take-off and when he pulled up the nose the aircraft didn't climb, the captain keeps telling him to push it forward instead of moving the throttle to full power. Aircraft overran the runway and ended up at a bridge plunging into icecold water. ASA261 Take of conversation between Alaska airlines and traffic control. Tape starts with Alaska Airlines contacting center with "We are in a dive here". After investigation the NTSB stated that the vertical stabelizer had a mechanical failure. CI611 Video of a crew who tried to land in the middle of a typhoon, they should have known better. If you look closely you can see the aircrafts banking to the right side before it touched down, they first of all should have attempted to land in this weather and secondly made a go-around when the aircraft started banking. DL191 Aircraft was in approach in the middle of a storm. You can hear the copilot telling his captain that he "saw lightening comming out of that one", the captain decides it's to late to get out. The get in the middle of a hail storm and the captain wanrs his copilot he is gonna "loose it all of a sudan". The copilot tries to push the aircraft up but was already to close to the ground. LAPA Cockpit tape. This crew didn't set the configuration right of the wings for take-off. While the computer keeps warning them about the error (you can clearly hear this thruw the whole tape) they just keep going. The aircraft enden up in the woods at the end of the runway. MD80 Video of an MD-80 wich lands with a much to heavy rate of descent. UAL232 Cockpit tape. Aicraft lost all hydraulics after an engine blow out. The pilots made it to the airport but they had to land using nothing but engine power to turn the aircraft and line it up. It landed to fast and ended up in a cornfield next to the airport at Sioux City. Good job of these pilots to make it to the airport, it has been tried thousands of times with the same condition in the sim, nobody made it so far. WAL2605 Cockpit tape. Pilots land on the wrong runway and impact construction vehicles. DL1141 Cockpit tape. Pilots are joking around with a flight attendant about the purpose of the cockpit voice recorder. 20 Mins later they are in take-off roll and have an engine failure, they overboost to late and the aircraft doesn't make by the end of the runway. AF320 The first fully automated flight video. The pilots wanted to demonstrate a fully automated ILS approach, followed up by a low fly by. However the aircraft continued a descent while the pilots where trying to get it up in the air again. Aircraft made a controlled crash, both pilots survived. I think the outcome of the investigation was a faulty altitude reading, not sure. |
