You might want to consider making the Driver License field go bye bye. That will be a decision making stumbling block for anyone to turn over to a new company outside the US that is not associated with any recognizable company.

I understand the purpose, but these days people are more reluctant than ever to parse out that kind of data unless its to a brand name company they recognize.
