Most of the time, it's a born-male with hormonal changes, either natural (rare) or medical (most often). Some get implants, but some find that with enough estrogen, they grow tits, which leads to erectile dysfunction (lack of erections).

So it means that the tranny can be also made by stitching the penis to a woman?

I've never heard of that. The one man I know who was a born-woman underwent hormanl therapy and had a penis stitched on after having hys female genetalia removed and stitched closed to create a perinium. He has a scrotum and prosthetic testicles and looks very male. (Matter of fact, I had no clue until he told me!!)

Edit: I'm not sure if that qualifies as a tranny. The usual definition of tranny as I know it (porn-wise, anyway) is a being with both male and female goodies. This guy no longer has female goodies (lost what little boob he had to begin with because of the hormone therapy).

First off. It's a boy who feels his brain is that of a woman and he has been born into the wrong body.

Depending on how close-minded you are or are not, that explains alot.

They then proceed to do whatever it takes to achieve their womanhood. Hormones (estrogen, birth control pills [yes, BC pills]), long hair, etc. Most cannot afforf to have their cock chopped. Plus is is very traumatic considering its something they've had their whole lives, it's something they use to jerk off (hey, they dont have a pussy/clit, so they gotta handle biz the only way they know how).

The hormones usually cause manly hair to stop growing. Thai transexuals don't ever shave I know. They don't need to. The hormones make their skin softer like a chick as well. Their muscles also become weaker and shaped like a womans. Some get hip implants to help their curves. Many get fake boobs. Some get nosejobs to have a girlier look. What they can NOT cover up are their foot and hand sizes. BUT many in Asia have small hands and feet genetically. I'm not 100% but I've heard of operations that can make the Adam's apple go away, etc.

In Thailand transexuals are called the 'Third Gender'. They are not allowed to serve in the military under classification of 'Mental Illness'.

What kmanrox said holds true as well. Many trannies don't feel as though they were born in the right bodies. Matter of fact, all of the transgendered people I know have that feeling, so they try to fix what's wrong. :)
