The maximum per keyword is actually a great feature that not many other PPC's do. Basically if you're willing to spend 20c per click but the next highest bidder is only paying 10c then you pay 11c. A nice little savings there. The reason you can't enter a fixed value is because they fluctuate in realtime based on max cpc and clickthrough ratio.
What doesn't convert is hard to tell to be honest because there's too many factors involved like the paysite, the keywords, the adcopy, etc... You need to tweak and see what works for you and your sites.
Best way to get lowest cpc is to be as dead honest as possible. If you're going after gay men for a dating site, when a surfer clicks, the tour they see better feature this exact niche. The adcopy should feature the keyword they searched for too and you should have no problem getting a 1% ctr.
