someone screwed up somewhere or something hasn't been updated? a spoofed ip address?

NO lots of ips do not have reverse dns set up

arin.net will tell you who owns the IP

64.122.0.0 -64.122.191.255 is allocated to integra out of portland oregon.

I can't find any ip's above 64.122.192.0 that are allocated. So if the ip is from 64.122.192.1 - 64.122.255.254 then it is probably spoofed or hijacked.

like kimmy said, hit arin.net and put the the full ip in and learn the full story.
