I need a small php form written.
 

It's an order form.

Process all fields, and dispatch an e-mail to me with all details and payment choice, and an e-mail (template) to the user with login url, user/pass, etc.

Some fields need to be *required* by the form, and some do not.

Redirect the user to the appropriate payment url. ie: /cc.html, /paypal.html, /epass.html.. whatever...

Something like that.

See, this is just a contact form. The difference will be the order form collects more information, sends e-mails to the user (based on a template like... /mail.txt), mails me with the complete form collection, and redirects the user to an appropriate page based on a radio button selection.

I should be able to edit any code in notepad to make changes if/when they become necessary.
