2257 requires full real name, birthdate, any other names performed under, and a picture ID.
Read the law, do it yourself. Now, releases for the pictures is another story. |
It also requires a physical address. NO PO Boxes. I think you should read it too as no one can lend you this. It's not a speel. It's a statement of compliance which includes the required contact information for the custodian of records.

It's only a legal document that could land you in prison. Fuck it, I think I'll just ask a bunch of lame webmasters for theirs.
