Its not the law, but as a rule I would say, if you can buy it in your local liscenced porn shop, your ok, if you cant, dont touch it

..so, if I live outside usa and have my servers in US, that statement would mean what to me?

In the case of pissing its not as extreme as scat, but it is discusting and humiliating to the average person, and not practiced by many people.

Any attorney specializing in adult will tell you its certainly over the edge and I would agree. But is it truly obscene? Most would say yes, but there would certainly be a vigorous debate in the jury room no doubt.
