It's not a hack. It is someone scanning your site for exploitable scripts.
You will know when it has been hacked.

Yes, they do. As do many other processors I don't care to mention.
A scanner won't scan for scripts that are not vulnerable. So if your scripts show up in a scan, chances are at one point in time they were vulnerable.
Update your scripts with the newest ones, and you will be ok until someone figures out holes in the newest ones. :2 cents:
