Anyway, does anyone have a page where I can get the template to the forms you need to fax in when you need to get your passwords recovered?

after all the legal shit they have been thru & are going thru because of faxed forms.... they STILL accept them???

They don't use this anymore. You can get your password reset and send to the email they have on file.
