It all depends

200 - 500 euro
Filming in Prague for about 12 years or so and rates are not standard.

In both Prague and Budapest, it is
400 euros - BG
500 euros - BGAnal
+20% agency fee

No other costs. Creampie, swallowing, etc, are up to the girls and sometimes whether or not they like the male model or enjoy working with you.

There is a bit of flexibility with the rates depending on how much work you are providing and the age/popularity/experience of the star. More so in Czech Republic.
