I used r8host.com and x10host.com to mix up my IP portfolio for link building - both have mysql and email. x10host closes you down for non activity once a month, which is a pain.

Neither have ever been down for me but I ended up paying $10 pa for much less restrictive hosting eg ukcheaphosts.com
