Does anybody know where I can get the jav. script code for a form like this where based on the users selections from 2 different boxes it goes to a different URL when the user hits "Continue"

Its PayPals, wouldent want to steal their code, plus its probably too confusing anyway. :-) I'm sure there is a simpler version out there somewhere.
