What causes something like this to happen?

Probably a similar mechanism to what caused this....

Ugh,those cysts are disgusting

It's called folliculitis. The hair will turn on itself and grow inward, usually becoming infected. Some cases are worse than others.

I would guess that isn't real being that it would represent several years of hair growth and the hair would ultimately be resorbed.
