I know not the type of celebrity but a great one is Cilla Black was originally Priscilla White. Nice change of name :thumbsup

IMDB exports their DB for free:

I am not sure if they list real names anywhere, you may have to hunt through those files a bit.

We have 1436 celebs with their real name field filled out. These are obviously only celebs that we chose to list with an AKA as their primary name since that is what they are typically know as. We've also got 3920 with AKA's if that helps you at all.
