If all their sex is protected, chances are that the customer will not contract HIV. Therefore, telling him could cause someone to die whereas not telling could allow both to live. It's a fucked situation either way, but I have to play devil's advocate..

The country should also be factored in. There are some 3rd world countries where 50% of the hookers are HIV+. In these countries, the guys already know the risk they are taking....
