Assuming such things are indeed accidents perhaps it'd be a nice idea to have a standing policy of offering any person pointing out such an error where affiliates would not receive credit $100? Seems like a win-win to me to prevent such things from somehow slipping through.

PORN.COM does something similar. On the individual DVD pages, if you click either the studio or series link, the links on the page you go to are without your webmaster id.

Pimproll owns them I think. Do they use affiliate cookies for tracking these days? I remember many years ago they didn't at all but they may have changed this. If not it might make this even more an issue...
