The pill travels directly to the penis area then a proximity sensor detects that the pill is in the correct location. It turns out that the guy was correct to run. His erection eventually subsided and he lived. So the first doctors who wanted to amputate were wrong. The wording is a little awkward but that's what I get out of this.
