Hi,
I've got a message from Penthouse that I need to fill that form : Form W-8BEN - Certificate of Foreign Status of Beneficial Owner for United States Tax Withholding.

I am not USA citizen. I live in UK right now and I pay my taxes in UK and Poland.
I need some help with understanding this form. I've never done it before.

- I can't see any instrustions what are those reference number(s)???

What is this? Should I check this box? Do I have to send them any documents, and If then what kind of documents would it be?

And if there is any representative of Penthouse / AFF reading this then I would have another question. May I send it signed and scanned by an e-mail? I don't have fax and common letter may take 2-3 weeks.
