They all pretty much work the same way in terms of SEO, and they will all fail the same way if the pages dont have any relevant content on them the directory structure isnt going to matter one iota $0.02

hmmm. I would go with www.domain.com/fruit/apple.html if you have a index page on www.domain.com/fruit/ with various content and links to other fruits like www.domain.com/fruit/pears.html

If you had to choose one of the 3, then #2. Otherwise, www.domain.com/fruit/apple/. No .html, i.e. the way most WP slugs are.

I don't think it makes a fucking difference. Google is smart enough that is really cares about "the content" and the words used in the actual page text. Here is a article from that says they will stop looking at meta tags too.

but i keep the tags in there for the other se's anyway....

If apple is a very competitive keyword, you will probably not be able to get rankings for domain.com/apple or any of the directories crap you mentioned. You need a stand-along website about apple, preferably with the best possible keyword domain like apple.com (.net and .org will do just as well for SEO). Adding fruit will do nothing. If you want to optimize for apple, you need to optimize for apple.

depends, to hit the apple keyword i would suggest to have dedicated pages for some types of apple and in that case you would use:

www.domain.com/fruit/apple/appletype.html

that would be the optimal imo.
