I tried a couple from hotscripts and they didnt work.

I just want something thats easy, like name, email address, select reason for contacting, message and thats it. It sends to my gmail account.
Where can I get something like this for under $20?

I remember doing this before php... we used form and formmail.pl or formmail.cgi

The problem isn't with the scripts that didn't work, it's with you or your server.

be careful installing some shit script, poorly coded one could be full of holes, which can be used by spammers to mail using your server... all will seem fine until one day out of nowhere your host will pull the plug on you...
