So they must use a bot. Such a bot would be rather simple. It would just have to spider twitter and add any user whose following-to-follower ratio is greater than one. Those tend to be the users who will follow you back.
When you look at the rather successful twitter marketers (who I suspect use bots), their following-to-follower ratio is near 1.
Also, a good bot would need to check back once in a while and remove the people who don't follow you.
