you register and offshore company, and then open an account in it's name in any bank - could even be any US bank

www.fletcherkennedy.com is a good site. Alot of PE Firms and Hedge funds used them to incorporate their offshore funds to avoid tax issues and hide bad assets... They can steer you to a stable bank and incorporate you. For the best offshre though, there is only one or two true Tax Havens that are left, Monaco and Gibraltar. Seychelles and Mauritus are on the rise too
