cj ultra help

hello all
This may not be the right place to ask, but I tried many other boards and no one seems to ewant to answer.
So if the following makes sense to you, feel free to help me.

I put the cj files on a sub folder "cj"
Where do I insert this tag:
<!--#include file="./cj/in.php"> is this the correct way to write it?
or is it:
<!--#include file="../cj/in.php"> or
<!--#include file="/cj/in.php"> or
<!--#include file="cj/in.php">

And also very importantly, where do I put it?
right after the <html> tag?

one more thing, I named my page index2.shtml is that ok?
