I called ibill today and they said everyone should have recieved the form and if not you need to call and open a trouble ticket immediatly.

i called ibill yesterday, the person i spoke with said not to fill in any forms and wait out this week to see what changes are made. Seems they have plans to help non-us based webmasters avoid all fees and all the visa registration crap!
