I am a clean freak. If I see a piece of lint on the floor I have to vacuum the whole thing.

It never gets a chance to get musty.

Add a few plants to help clean the air.
                        Clean often.
Do open windows every so often to help air it out.
Remove any carpet if you have it, or at very least get it shampoo machine and clean it.
If you have drapes get those cleaned as well. (they have companies that do carpet and drapes)
If you have a fridge, remove all items and throw away bad stuff. Wash inside with a solution of water with about 5 to 10% bleach then put stuff back in.
If you smoke in your office, stop and smoke outside.
Get some fabreeze or something and spray any soft materials left in your office including chairs.
Use some dissinfectants or handwash with same solution as fridge any trash cans.

plants are key, fresh flowers are great, open the windows, get a dehumidifier (you'll be shocked how much water it will put out of the air), get some air flow going

clean the carpets, i dont care how clean they look, watch the shit you pull off of em.

opening the window always works to help things air out too.

I have a fan that I keep going on low, just to circulate air through. On warmer days I crack the window a bit. I'm not a fan of incense because I find it actually adds to the mustiness over time.

Vacuum often! Sprinkle baking soda on the carpets before you vacuum a couple times a month! That's about it.

Get a Lampeberger. They are sold at cigar stores because they do such a good job a cleaning the air. Another webmaster who smokes cigars recommended it and it's great for dog smells. I get my oil here:

                                      http://www.maisonbouquet.com/

Mix it with 91% (make sure it's *91%*) rubbing alcohol - depending on the scent I do half/half or 1/3 oil, 2/3 alcohol.

If it's truly musty a dehumidifier will work wonders. Get one that you can channel outside if it's really damp. I had one in my basement in the burbs and they turn off when all the moisture is out of the air - mine didn't turn off in 5 years, lol.
