I don't believe there is any official "file docs here"
It's more what you have to keep readily available at your location, and how you have to keep it.
To my knowledge, ID... Aliases... most content providers I've worked with also keep blood test docs with these.

Yes, and they are usually found in the A section of the phone book. Though most in most cities will not be familiar with the law nor the Internet. Yet any competent one will be able to explain it to you and write up what you need.Comment
