There's far more important things in life to be concerned or worried about than if the ex's wife likes you or not.

Make-up can cover the birthmark if you're self conscious of it. I personally don't see anything wrong with it. It's not like you have a huge purplish birthmark that covers half your face. Your nose is fine too.
