Here's my take.

I'm assuming he's a reasonably stable guy, so it's going to have to be something physicaly to induce heart attack sensations.

That's narrowed out Visa, etc etc.

I'm betting it's going to be a luxury electrical item... perhaps a new cappuccino machine, or an electric razor.
Seems to fit all bills, including luxury and TV adverisments.
