Anybody know why my icons are displaying like this?

My Windows XP icons are showing this black lightning bolt as well as showing as "hidden." I was wondering if anybody has ever seen this before? I took a screen shot to show you what I mean:

    Any ideas?

I recovered files from an external hard drive, and thats when the issues started to occur. I have played with every setting and still cant figure it out.

I have been to Microsoft help, annoyances.org. I searched for icons, icons not displaying, thumbnails error, icons error, windows xp lighting box, black lightning box, etc.

          No explanation yet.

No new installs.. just recovered the files from an external hard drive and they came out like that. New icons, like a new word document look totally fine.

                      Download Tweak UI.

                      Click Repair.
                      Select Rebuild Icons.
                      Click 'Repair Now'.
