Depends which way the wind is blowing. Being 50 miles away will only protect you from the initial blast wave.The dust cloud formed by the explosion could be dispersed hundreds or thousands of miles. As an example, the dust clouds from Africa that somehow make it all the way to the US.

Also, if your water/food sources are contaminated you will still get radiation poisoning through ingestion rather than inhaling

I don't know. I saw their IEDs (Improvised Explosive Devices) from london and I was making similar, if not better bombs in highschool.

But the fact is they aren't stupid. Let's say the devices aren't all the sophisticated. They are choosing the *right* locations to deploy them.

They completely disabled London with a few well placed bombs. They're not stupid and if people keep believing that they are then we're in for a huge war very soon.
