My current temp designer sucks. His incompetent ass had a due date on Monday, and it is now Wed and he still hasn't come through. I didn't pay him yet either, but he still was supposed to have a rough draft for me. Anyway, enough about him, let's talk about the project.

I need a non-adult ONE and a HALF (index and thank you page) page site made for today. I will pay you $75 for it via PayPal. I can send half after the rough draft and half after it's completed, or the whole thing after completion. It's totally up to you.. Just follow the instructions below and all will be good.

Here's what the project needs:

It is a 3 step signup process. Very smooth and quick.

Step 1: MyFreeOnlineOffers.com as an image welcoming the user to the site, and tells them "You are THREE steps away from receiving as many free products from the list below, by filling out just ONE form, all for FREE."

Step 2: The form - My programmer will make it work, I just need the form to be there. It requires the following fields:
First Name
Last Name
Address
City
State
Zipcode
Email address
Birthday (month - day - year)

Step 3: Here's where the most work of the whole project comes into play. This is the place where all of the offers will be listed. I need 25 checkboxes with 15 of them randomly pre-selected. Over each checkbox will be an image of the offer in a 100x100 pixel image. I'll supply the images for you. Aside from the checkbox options, there also needs to be 5-6 choice button options, allowing them to decide between two offers (ie. Toshiba or Gateway). Same 100x100 pixel images on that.

Then a checkbox at the bottom for them to agree to the terms, and a Thank You Page for them after they fill out the form. The thank you page should just say "Thank You.. Your information is being processed. Come back soon to check for more offers!"

That's it.

ok here is a rough draft, I was a little confused about the request for
