Lob. Looking over your requirements there is nobody on this board that meets all of those.

However, I can reccomend you to a few who come close.

Techiemedia.net although they don't have control panels or live irc chat. They do have an excellent ticket system that works well. Most tickets there are answered within minutes taking the work of a control panel out of your hands. They run FreeBSD.

Phatservers.net Same as above and these guys know thier stuff.

I am not affiliated with either of the above. However I do adult hosting consultations and I do know a bit about the inner workings of both companies.

I hope this is the kind of recommendations you were looking for.
