Need a host that will let me run SSI on .HTM & Unique IPs - GoFuckYourself.com

I already have hosting and am very happy with them. I am setting up another business project and need to have a different hosting company (long story). I need:

1) Not a whole bunch of bandwidth to start, but would like it to be reasonably priced and with the possibility of needing more in a couple months.

2) Unique IPs. Are they any extra and how many do I have to get at once? No 130.94.x.x or 216.240.x.x - there is nothing wrong with them, I just can't use them IPs or DNS.

3) 24 hour live technical support. This means there is a TECHNICIAN THERE to ANSWER the PHONE.

4) Some sort of a reasonable uptime guarantee.

5) They must allow me to use SSI on .htm pages using .htaccess (or any other reasonable means).

6) US Based Hosting that has existed for at least a year with no arbitration clauses. I do not like to do business with companies with such clauses.

7) Oh and no charge for extra domains.

A control panel is nice too, but not necessary if you can add domains in a timely fashion.

I know that is a long list, but I am confident someone here will have reliable hosting

Both include SSI, PHP, .... and you can have multiple IPs.
