It really doesn't look fake. I mean, the people don't look like they're in much of a hurry probably because they're trying to think of how to fix his problem! That's not something you can just pull out, and have everything be fine...

Another interesting detail is the pants ripped around the edge of the pole... entering his leg. Looks like the guy might have fallen off a building and landed on a pole of some sort.

I read a story in a medical book once about a guy who hit a stop sign and the 6" round wooden signpost went through the windshield and then though his chest and missed the vital organs. He drove himself to the emergency room and honked his horn till someone came out. They had to cut the post in front and behind him so they could get him out of the car. The post kept all the blood inside him. They had several pictures taken during the ordeal and then guy was wide awake although you could tell he was in shock. Wouldn't want to trade places with him.

It could be a saw mill accident. My girlfriends father used to work in one and he told me some crazy stories of logs/lumber impaling guys right through the stomach. :uhoh
