Ban IP address - GoFuckYourself.com

I have this MOFO using a contact form in my site just to send stupid messages, his IP seems to be static. What's the code to ban him with .htaccess?

Well, I was typing something when my power went out for a split second, here's the code, this isn't php this is htaccess, this was the only way I could get the brakets to show up!:
