Looks like your right. I dont get this tax form i havent cashed 6k in checks by them this year. I guess ill just report all the checks cashed.

You can contact them if you think it's wrong, they can send a corrected one if it is. But your cutting it a little close for taxes this year.

Is it a 1099? Those are sent to the IRS also. So if it's incorrect I would request a corrected one from them to be sent to the IRS.

It's possible another company uses that name too but who knows.

it says combined tax statement for forms 1098, 1099, 5498 for tax year 2021
